Jed was diagnosed with stage 4 prostate cancer and was later referred to Florence Nightingale Hospice for palliative care.
He was put under the care of the community team and began to receive visits at home from Cleo, a Clinical Nurse Specialist, who put in place a package of support measures to help manage his pain and help him keep his independence.
Jed regularly visited the Hospice’s Lymphoedema Clinic where the team designed a personalised treatment plan to help ease pain and manage the swelling, which had been caused by Jed’s illness. The Hospice’s physiotherapy clinic also created a plan to help him to stay mobile, enabling him to do the simple things he loved like gardening.
Unfortunately, the spread of Jed’s cancer progressed quickly, and Jed’s family focused on making him comfortable at home with the support of the FNH@Home team.
Reflecting on the level of care the family had received, his daughter Vicki said: “Until we lost Dad I didn’t know about the huge package of care and support available at home, and the support given not just to Dad but to the whole family, we couldn’t have done it without them.”
